ABSTRACT:
Malabsorption syndromes are a mixed bag of disorders, sharing the common phenotype of nutrient malabsorption and frequently manifesting with similar gastrointestinal and systemic symptoms. Conventional diagnosis based on clinical or histological findings alone can potentially result in misdiagnosis or delayed treatment. A new diagnostic model that uses a combination of clinical features with mucosal biopsy findings offers a more exact and immediate diagnosis. To determine the diagnostic value added to clinical presentation when incorporating mucosal biopsy findings in the distinction and management of a number of malabsorption syndromes. This article is an update of the investigation on clinical, endoscopic and histopathological aspects of malabsorption. It describes plays by biopsy sampling, processing and interpretation as well as disease specific correlations via case-based illustrations. Thus, potential diagnostic tools available in the future including digital pathology, molecular markers and AI-supported discipline of histology are also considered. Integrative approach improves the diagnostic accuracy especially in cases such as celiac disease, tropical sprue, giardiasis and autoimmune enteropathy. Clinical-pathological correlation enhances the severity stratification, assists in determining treatment and clarifies doubtful cases. Multispecialty evaluation further clarifies the diagnosis in complicated presentations. Novel technologies add to the appearance of classic histology, with the predictive and personalized promise that comes along with it. Clinical features and mucosal biopsy interpretation together enhance the accuracy of diagnosis of malabsorption syndrome significantly. Such an integrated strategy promotes individualized care, enables targeted intervention and reflects personalized medicine.
